The MTXL is reading the windows-registry to get the install-path of a game. If an AV-program or something else is keeping the trainer from reading the registry, the trainer doesn't know how to start the game. In addition it's possible that there's a problem getting the right registry-value too. Due to the fact we don't own this version of Civ3 ourselves, we cannot test it and need to trust the guest-member we've instructed in getting all needed information and who have tested the MTXL-version as first. We know that the start-button is working on some Civ3Complete versions and for unknown reason it isn't working on some other versions (although it should always be the same). However, you should run the game yourself and the trainer should work too, because the start-button is only an additional feature and not needed to use in the MTXL.
